We have provided a fillable pdf application form for your use. Download and type in the information, print and sign the form, sign the application and mail or deliver to EJA. Remember, you cannot save the filled form; it must be printed.

Birth certificate
Current photo of child
Financial Information
Health and Immunization records
Record of recent physical exam
